Acetyl L-carnitine (ALCAR) is the more bio-available form of L-carnitine, a quaternary ammonium compound, with which it is interchangeable. ALCAR and L-carnitine act as a transporters of fatty acids into the mitochondria and help with energy production. It is also has regulatory effects on Acetyl-CoA, a crucial compound that has a wide range of roles in energy production pathways. ALCAR has been well researched and is being used in the following clinical conditions and areas:
- Age-related cognitive decline.
- Aging.
- Cardiovascular disease.
- Alzheimer’s.
- HIV and AIDS.
- Athletic performance.
The area it is most commonly used is for the prevention of age related cognitive decline, usually on conjunction with Alpha lipoic acid. There is excellent evidence it works well in the rat model and now also in large mammals such as dogs. Human studies are currently underway and we await the results eagerly. It has also been shown that numerous biomarkers of the aging process have been significantly reduced by administering this combination, including the amount of DNA damage in many tissues. There is good evidence to suggest L-carnitine is useful for people with cardiovascular disease. It seems to reliably improve exercise capacity in people with heart disease (angina and heart failure) and improves the distance people with blocked leg arteries can walk (intermittent claudication). The data for Alzheimer’s are so far disappointing but there may be some benefit in younger people affected by this terrible process. ALCAR can produce a stimulatory effect and insomnia. Some people are more prone to this than others and dose adjustment may be required.
